心筋SPECTの診断精度

要点
  • 負荷心筋血流シンチの冠動脈疾患の診断能は、冠動脈造影所見との対比において、症例ごとの評価では感度は80-90%程度、特異度は80-95%程度、冠動脈枝ごとの評価では感度は70-90%、特異度は80-95%程度とされている。
  • 心筋血流SPECTと冠動脈CTの所見に乖離を生ずる場合で多いのが、冠動脈CTにおいて狭窄があり、SPECTで血流が正常の場合である。解剖学的な冠動脈狭窄と生理学的な虚血の出現の差異に注意する。
 

冠動脈造影所見との比較

  • 負荷心筋血流シンチの冠動脈疾患の診断能についてはこれまで多くの報告があり、冠動脈造影所見との対比において、感度と特異度は以下のとおりである[1, 2]。
    • 症例ごとの評価では感度は80-90%程度、特異度は80-95%程度、
    • 冠動脈枝ごとの評価では感度は70-90%、特異度は80-95%程度
  • %uptakeや洗い出し率の計測、circumferential profile analysisといった定量的な評価法の導入により診断能の改善が得られるとの報告もある[3-6]。
  • 運動負荷では運動耐容能が予後指標となることが知られているが、運動量が不十分であると診断感度が低下する[7, 8]。
  • カルシウム拮抗剤、亜硝酸剤、βブロッカーなどの服用も検出感度に影響をおよぼすが[9]、薬剤投与時の虚血の程度を評価する目的の場合には薬剤投与下での負荷検査がなされる。
  • 冠動脈造影による狭窄度の定量的評価に比較して負荷血流SPECT所見は局所血流予備能の測定値とより相関するとの報告があり[10, 11]、形態情報とは異なる虚血評価が可能であると考えられている。
  • small heartの症例においては診断能が劣るとされる[12, 13]。 検査システムの空間分解能があまり高くないことに関連すると考えられる。small heart は心電図同期SPECTで評価を行う際の誤差要因のひとつでもあり注意を要する。
 

冠動脈CTとの比較

  • 冠動脈CTの狭窄病変の検出能は冠動脈枝ごとの評価で感度は80-95%程度、特異度は85-95%程度とされ、陰性的中率がきわめて高い(97-99%程度)ことが知られている[14-17]。ただし、石灰化などで評価不可能な部位が除かれている点には注意を要する。
  • 冠動脈CTが正常所見で、SPECTにおいて血流低下を認める場合は、SPECTの偽陽性(アーチファクト等による)もしくは冠動脈閉塞後の自然再疎通やスパズム解除後の心筋ダメージの存在が原因として考えられる。このパターンの乖離の頻度は比較的低く、冠動脈CT正常所見例のうちの10%程度であるとされる[18]。
  • 負荷心筋血流SPECTと冠動脈CTの所見に乖離を生ずる場合で多いのが上記とは逆のパターンで、冠動脈CTにおいて狭窄があり、SPECTで血流が正常の場合である。冠動脈CT異常例の半数程度で見られるとされる[18]。このパターンの乖離の原因として、多枝病変例での各病変部位による虚血の程度がほぼ同等で(balanced three-vessel disease)、相対的なカウント分布で判断するSPECT画像上で異常として認識されない場合もあり得るが、頻度は多くない。むしろ、冠動脈狭窄があっても実際に虚血・血流低下として影響していないことが主な原因と考えられる。このような症例の扱いをどうするかが今後の検討課題である。
