日本における心筋SPECT多施設研究

要点
  • 画像診断による全国規模の多施設研究としては、日本人においてはJ-ACCESS予後評価研究が最初である。
  • 4629例の症例登録とその3年間の経過観察で、心筋血流と心機能が心事故の予測因子となることが明らかとなった。
  • 背景因子としては、糖尿病の合併と、慢性腎疾患の合併が重要な心事故予測因子であった。
 

J-ACCESS研究のの方法

  • 2001年10月より6ヶ月間、全国117施設で、4629名を登録した。
  • 登録基準は、負荷・安静心筋SPECTが施行され、QGSによる心電図同期SPECT解析が可能であることとした。
  • 予後調査の研究期間は3年間であった。
  • 心筋血流の評価には心筋血流イメージング製剤である99mTc-tetrofosminを用いた。
  • gated SPECTを用い、心筋血流と併せて 心機能解析も行った。
  • 負荷時の欠損スコアSummed Stress Score(SSS)、安静時の欠損スコアSummed Rest Score(SRS)、両者の差のスコアSummed Difference Score (SDS)を 定量評価の指標とした。
 

J-ACCESSの主要解析による結果

  • SPECT施行後60日以内の早期血行再建を除外し、4031症例について主要イベントを中心に解析を行った。
  • 心死亡と、非致死的心筋梗塞をハードイベント、入院を要する重症心不全を加えた心事故を主要イベントとして解析した。
  • ハードイベントは2.4%/3年、主要イベントについては4.3%に認められた。
  • ハードイベント群では無イベント群と比較して男性の割合が多く、心筋梗塞の既往が多く、糖尿病が多いという特徴があった。心筋血流SPECTのパラメータではSSSが高く、SRSが高く、左室駆出分画は低く、EDVとESVはより高値であった。
  • Kaplan-Meier解析では、SSSに基づいて正常、軽度欠損、中等度欠損、高度欠損の4群に分けると、中等度以上の欠損で心事故発生が高いことが明らかになった。
  • 正常SPECTでは心事故発生率が低いことは欧米のデータでも指摘されていたが、J-ACCESSデータから見ても、主要イベントは0.8%/年、ハードイベント0.5%/年であった。
  • これに対して、SSSで中等度および高度の異常群では、3年でそれぞれ5.5%および9.2%であり有意の差が見られた。

J-ACCESSから学べる事柄

  • 日本人の心事故発生率は欧米での研究と比較して低い
  • 心事故発生には、心機能に関連する収縮末期容積、駆出分画と負荷時心筋血流欠損が重要な因子である
  • gated SPECTにより、リスク層別化が可能であり、とりわけSSS<4ではハードイベント発生が0.5%/年と低い
  • 背景要因のうち特に糖尿病と慢性腎臓病が予後判定の重要な予測因子である

J-ACCESS 2,3,4

  • 無症候の糖尿病を対象とした関連研究 J-ACCESS2が実施され、最終報告がなされている。
  • 慢性腎臓病(CKD)を対象とした関連研究 J-ACCESS3は2009年より開始し、現在中間報告が出されている。
  • J-ACCESS 4研究は血行再建における虚血の減少と心事故は性の抑制に関して実施中である。
